Open an Account

Opening an account with HyGen Pharmaceuticals, Inc. is easy.

How to open a Customer Account:

Step 1 - Download and print an applicable copy of our Customer Application: Retailer/Physician or Wholesaler/Distributor.

Step 2 - Complete the application in its entirety.

Step 3 - Send it to us via fax (425-451-8964) or email (info@hygenpharma.com) along with a copy of your Resident State Board of Pharmacy License.

Step 4 (WA Customers Only) - Include copies of your Resale Certificate and Prescription Drug Exemption Certificate.

Once we have received your completed application, it will be reviewed internally. You will be contacted if any further information is needed in order for us to be able better service you as our valued customer. Once this process is completed, you will be ready to place your first order within 24 hours.

How to open a Vendor Account:

Step 1 - Download and print an applicable copy of our Vendor Application: Manufacturer or Wholesaler/Distributor.

Step 2 - Complete the application in its entirety.

Step 3 - Send it to us via fax (425-451-8964) or email (info@hygenpharma.com) along with a copy of your Resident State Board of Pharmacy Distributor License and Washington Out of State Distributor License.

Once we have received your completed application, it will be reviewed internally. You will be contacted if any further information is needed in order for us to be able better service you as our valued vendor. Once this process is completed, we will be ready to place our first order within 24 hours.
