Open an Account

Opening an account with HyGen Pharmaceuticals, Inc. is easy.

 

How to open a Customer Account:

Step 1 - Download and print a copy of our Retailer/Physician Customer Application.

Step 2 - Complete the application in its entirety.

Step 3 - Send it to us via fax (425-451-8964) or email (info@hygenpharma.com) along with a copy of your Resident State Board of Pharmacy License.

Step 4 (WA State Customers Only) - Include copies of your Resale Certificate and Prescription Drug Exemption Certificate.

Once we have received your completed application, it will be reviewed internally. You will be contacted if any further information is needed in order for us to be able better service you as our valued customer. Once this process is completed, you will be ready to place your first order.

 

How to open a Vendor Account:

Step 1 - Download and print a copy of our Manufacturer Vendor Application .

Step 2 - Complete the application in its entirety.

Step 3 - Send it to us via fax (425-451-8964) or email (info@hygenpharma.com) along with a copy of your Resident State Board of Pharmacy Distributor License and Washington Out of State Distributor License.

Once we have received your completed application, it will be reviewed internally. You will be contacted if any further information is needed in order for us to be able better service you as our valued vendor. Once this process is completed, we will be ready to place our first order within 24 hours.


A
